Schedule 5 Tax Form: Details of Dependants in Canada

What is the Schedule 5 tax form used for?

You use the CRA’s schedule 5 when you want to claim the amount for an eligible dependant, the amount for infirm dependants age 18 or older, the caregiver amount and the allowable amount of medical expenses for other dependants.

You indicate the information about the dependant on the CRA schedule 5 tax form and claim the amount on the appropriate line. 

What amounts can I claim? 

  • Amount for an eligible dependant: Consult the T1 General Guide to see if you are eligible and claim the amount on line 305.
  • Amount for infirm dependants age 18 or older: Consult the T1 General Guide to see if you are eligible, calculate the amount on the federal worksheet and claim the amount on line 306. 
  • Federal Caregiver amount: Consult the T1 General Guide to see if you are eligible, calculate the amount on the federal worksheet and claim the amount on line 315. 
  • Allowable amount of medical expenses for other dependants: Read the instructions on line 331 in the T1 General Guide and claim the amount on line 331.
